PERMISSION FORM FOR UNESCORTED MINORS PARTICIPATING IN CAROLINA MOUNTAIN CLUB ACTIVITIES

By signing this from I give permission for ___________________________________ to participate in Carolina Mountain Club activities.
I understand that participating in and traveling to and from this hike or activity may expose the minor named above to a variety of hazards and risks that could result in serious personal injury, illness, or death. By signing this sheet I take full responsibility for the minor named above and his/her safety and release the Carolina Mountain Club from all claims and liability. I also hold harmless all other participants of this activity even if harm is caused by negligence in any way connected with this outing, including travel to or from the hike or activity.
I understand that the minor named above must abide by the rules and regulations of the Carolina Mountain Club concerning hiker safety, to follow the instructions of the leader(s), and not to become intentionally separated from the group, either ahead or behind.
